The Hospice Care Team

Hospice Care is delivered by a team of compassionate professionals who provide an increased level of support, allowing the patient to spend meaningful time with loved ones and live each day with dignity.

CONSISTENT CARE

Members of the Hospice Care Team visit the patient regularly and provide additional care or other services when needed.

ANSWERING THE CALL

If an issue arises after regularly scheduled visits, nurses are available on-call 24 hours a day, seven days a week.

PERSONALIZED APPROACH

Intrepid USA collaborates with the patient, their family, and their healthcare provider to develop a personalized care plan to match their unique needs, resulting in reduced hospitalizations and enhanced quality of life.

The Hospice Care Team includes a variety of dedicated experts and hospice coordinators, each one specifically focused on improving quality of life by addressing the physical, emotional, and spiritual needs of the patient and family.

Hospice Care Team Members

Attending Physician

A physician of the patient’s choice directs the care as the Attending Physician. They will co-certify prognosis, along with the Hospice Medical Director, and approve and direct the plan of care based on the patient’s individual needs.

Hospice Medical Director

The Hospice Medical Director co-certifies the prognosis with the Attending Physician, in consultation with the referring physician, and leads the overall plan of care for hospice services.

Nurses

Hospice Nurses include RNs and LPN/LVNs. In collaboration with the patient and family, they assess the patient’s needs and develop a plan of care to manage symptoms, increase comfort, and improve quality of life.

Social Workers

Social Workers assess the psycho-social and emotional needs of the patient and family. They provide support and education and connect them with available resources to help meet those needs.

Hospice Aides

Hospice Aides work under the direction of the nurses. They attend to the personal care needs of the patient.

Spiritual Coordinators

The Hospice Spiritual Coordinators assess the spiritual needs of the patient and family. They provide counseling and support, regardless of religious or spiritual preferences, and participate in memorial services and support groups in the community.

Bereavement Coordinators

Hospice Bereavement Coordinators provide 13 months of support to the patient’s family members. They provide resources and referrals within the community and offer short-term counseling for family members, as needed.

Volunteers

Hospice Volunteers are available to provide companionship and non-medical support. At Intrepid USA, we try to match patients and volunteers according to hobbies and other similar interests when possible. Our Volunteers may join a patient to watch a favorite sporting event or help write cards for loved ones. There are many ways in which Hospice Volunteers add comfort and improve quality of life during hospice care.

Other Team Members

The hospice team also works with pharmacists, dietitians, and physical, occupational, and speech therapists. They may be consulted for short-term therapeutic assistance that can help achieve hospice goals within the plan of care.

When is it time for hospice?

The best time to learn about hospice is before you need it. By contacting hospice early in the diagnosis, the patient and family are assured to receive the maximum benefit from those services.
